- Student Information Sheet
- The following forms won't be needed by every student only fill one out if they apply to you.
- In order for the school to dispense Tylenol or Ibuprofen you must sign and return:Over the Counter Medications Form & Medication Log
- Does your student need to take a prescription medication at school: Prescription
- Does your student use an inhaler at school: Inhaler
