Out of District Application and Renewals: CLICK HERE**
Out of District Students must submit an application (along with any other required documentation) each year.

**The link to the out of district application has a "Submit Form" button that only works if you open the PDF in Adobe Acrobat. It will not submit if you are in a web browser (Chrome, Firefox, etc.). An alternative would be to fill out the form in the browser, download the completed PDF, and email it to kwalters@usd268.org

PARENT/GUARDIAN DIRECTIONS FOR UPDATING STUDENT INFORMATION IN ENROLLMENT

INTRODUCTION
Enrollment Express is a data collection system integrated into PowerSchool. Enrollment forms will be shared with you via the Parent Portal in PowerSchool

PARENT PORTAL ACCESS
Go to https://usd268.powerschool.com on your computer or laptop browser.

Enter your Username and Password you created and click Sign In.

If you do not remember, select Forgot Username or Password? Be sure to enter the email address you associated with the account, and you should receive link to reset in your email.

SETTING PREFERENCES
Select Preferences if you would like notifications when your Forms have been approved (residency verification and guardian information).

ENROLLMENT EXPRESS (FORMS)
You will see a new "Forms" link on the left-hand navigation bar within the PowerSchool Parent Portal. This screen will show you the forms that will need to be entered/updated for your student. Next, click on the "Enrollment" tab to locate the forms required for your student. Enrollment forms will be made "active" starting July 1. Click the blue hyperlink for the first form . For each form, verify the information is correct or make any necessary changes.

When you are done with each form, click the blue Submit button (even if there are no changes). After a form is submitted, you will see a green leaf.

Then navigate to the next tab to complete the next form.
Please complete and submit all forms. You will need to repeat this process for each child in the portal.

1. Certified state copy of a birth certificate. If you need to apply for a birth certificate click here: Birth Certificate Application

2. Copy of current immunization record. Refer to this link: Immunization Requirements for the immunization requirements for the 2022-23 School Year. Refer to this link to download the KCI form: KCI

3. PreK-3rd: Complete Health Assessment filled out by your child's Medical Provider. This may be done up to 12 months prior to school entry. Refer to this link to download the Health Assessment form: Health Assessment

5.Refer to this link: Inhaler Permission Form to download the Inhaler Permission Form which permits students to carry an inhaler.
